ADULT ACCIDENT WAIVER, RELEASE OF LIABILITY, INDEMNIFICATION & MEDIA RELEASE

ASSUMPTION OF RISK

I understand that participation in the St. Petersburg Festival of Dance and all related activities—including, but not limited to, classes, rehearsals, workshops, performances, camps, intensives, private instruction, choreography, and travel to and from the event (collectively, the “Activities”)—involves inherent risks.

I knowingly and voluntarily assume all risks, whether known or unknown, foreseeable or unforeseeable, including but not limited to personal injury, illness, disability, death, property loss or theft, natural disasters, acts of God, and risks arising from the negligence or carelessness of the Released Parties, dangerous or defective equipment, premises conditions, or liability without fault.

MEDICAL REPRESENTATION

I represent that I am physically and mentally capable of safely participating in the Activities. I have not been advised by a licensed medical professional to refrain from participation and have no medical condition that would prevent safe participation.

RELEASE AND WAIVER OF LIABILITY

In consideration of being permitted to participate, I, for myself and my heirs, executors, administrators, successors, and assigns, hereby waive, release, and forever discharge the St. Petersburg Festival of Dance, and their respective directors, officers, managers, employees, contractors, instructors, volunteers, agents, sponsors, affiliates, and event organizers (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind, whether arising from negligence or otherwise, related to my participation in the Activities, including travel to and from the event.

IDEMNIFICATION

I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, losses, liabilities, or expenses (including reasonable attorneys’ fees) arising out of or related to my participation in the Activities.

MEDIA CONSENT

I grant permission for my image, likeness, voice, and performance to be photographed, recorded, and used by the St. Petersburg Festival of Dance and its affiliates for promotional, marketing, educational, or archival purposes in any medium, without compensation.

GOVERNING LAW & SEVERABILITY

This agreement shall be governed by and construed in accordance with the laws of the State of Florida. If any provision of this agreement is held unenforceable, the remaining provisions shall remain in full force and effect.

ELECTRONIC SIGNATURE

By signing below, I acknowledge that I have read this agreement in its entirety, understand its contents, and voluntarily agree to be legally bound by it. I understand that my electronic signature is the legal equivalent of my handwritten signature.
